E-mail the Resource Center, and provide the name of the user who needs access to the form. You will get an e-mail reply telling you when this access is added to your registration. 

I am NOT registered on MMA's site and need access to the P & C Claims Form

Click to Register  Fill in this form and check off "Prop and Casualty" in the Security Group field. If you also need access to the Workers Compensation First Report of Injury, hold down the "Ctrl" key and check both of them off at this time. You will receive an e-mail confirmation when it is activated.
