Interactive Forms

In our Member Resources area, interactive forms can be identified by the Interactive Form symbol located beside links to PDF documents that have active fields so that  members can type into and print from their computer.  This allows the printed form to have the same format/appearance as the form seen on the screen or in its print version. Some of the forms provide various mathematical computations as well.

How can I use Interactive .PDF forms?

When the hand Adobe Reader's "Hand" icon turns cursor into a text bar, you may type into the field and then tab to the next one. Some interactive fields have a slight gray frame around them so that they are easier to find. 
You can use the Adobe menu's disk icon to "Save a copy of the file" to save a copy onto your machine. You can then use it as often as you like without going online.

Be sure to check our site periodically to determine if the form has been updated due to changes in law.

The entered information will not be saved if you are using Adobe Reader.

If you are having problems opening a PDF document, the usual cause is an older version of Reader. To use interactive forms, you should have 4 or higher. Go to Adobe and download a free updated version of Acrobat Reader by using the link to the right.
