Community Health Worker
Full Time – Must be bi/multi-lingual
The Community Health Worker assists immigrant new Mainers with young children or who are expecting, to have access to the services they need to grow, develop, and thrive.  
    
Responsibilities: 
•    Work with immigrant families, in York County, to develop and implement effective and robust plans for outreach, care coordination, and referrals to services, with a focus on parents who speak English as a second or third language and who have recently moved to Maine.  
•    Educate parents/caregivers and expectant parents, in their own cultural context, the importance of developmental screenings and developmental milestones with the goal of supporting the young child(ren)’s healthy development and ensuring they are prepared for success in Kindergarten. 
•    Connect young children to preventative health and early intervention services and assist parent/caregivers in accessing and understanding what to expect during Well-child Visits and other medically necessary services with primary healthcare providers and when needed, Child Development Services. 
•    Complete CHW Core Skill Training.
•    Work with and receive referrals from other YCCAC departments and programs already assisting immigrant new Mainer families.  

Qualifications:
•    Bachelor’s Degree, Associate Degree or professional credential in public health, community health or related field required; Bachelor’s degree preferred. Directly related work experience may be substituted.
•    Proficiency in English (spoken and written) as well as at least one other priority language (Portuguese, French, Lingala); Required.
•    Community resident with good knowledge of the resources available within the community.
•    Ability to quickly establish trust and respect with participants and co-workers.
•    Sensitivity to challenges faced by families living in poverty and/or food insecurity.
•    Strong grasp of MS Office Suite preferred.
•    Prior experience as a Community Health Worker, health coach, or outreach worker; desired. 
•    Annual driving record checks will be completed and employee must maintain a valid license.
